HCPCS Modifier FS

Description
Split [or shared] evaluation and management visit.

Guidelines and Instructions
This new modifier (effective for dates of service on/after 1/1/2022).

Use with claims for split (shared) visits performed in facility settings and split (or shared) critical care visits. Split/shared visits can be reported for new and established patients, initial and subsequent visits.

The modifier does not apply in an office practice setting.

Use this for shared or split services between a physician and non-physician practitioner, including critical care. Documentation in the medical record must identify the two individuals who performed the visit. The individual providing the substantive portion must sign and date the medical record.
